  • In-home counseling, intensive supervision, electronic monitoring and limited substance abuse services provided through VJCCCA funding through contract services.
  • Accountability conferencing services (Restorative Justice) accessed through local dispute resolution center.

Forms

  • Intake Information Sheet -- This form is to be completed for all intake requests.
  • Confidential Info Notice -- This form keeps contact information confidential specific to Protective Orders and Support or at the request of any complainant.
  • Custody or Visitation Affidavit -- This form accompanies a request for Custody or Visitation of a child and for Protective Orders that temporary custody is requested.
  • Petition for Support -- This form is to be completed when requesting support for a child or children.
